NAD+ and Methylene Blue: Evidence, Interactions and Risks
Does combining NAD+ with methylene blue improve energy or cognition? What human studies show, where FDA approval applies, and why medication review matters.

NAD+ and methylene blue are sometimes offered together for energy, focus or longevity. The published studies reviewed here do not establish that this combination delivers those benefits. Evidence about one ingredient cannot demonstrate that adding a second works better, and methylene blue introduces medication risks that an NAD+ discussion alone can miss.
Start by identifying the actual products. NAD+, NMN and NR are different interventions, while methylene blue is a drug with specific medical uses. A clinic's combination name does not turn these into a treatment with its own demonstrated efficacy.

See WillowWhat methylene blue is approved to treat
The U.S. prescription product PROVAYBLUE is an intravenous methylene blue injection approved for acquired methemoglobinemia in adults and children. This is a disorder affecting hemoglobin's ability to carry oxygen. Its approval does not establish an oral wellness product or an NAD+ combination as effective for fatigue, memory or aging 1.
The distinction concerns the product, route and purpose together. An online liquid, capsule or research chemical is not interchangeable with the approved injection simply because the ingredient name matches. Likewise, prescription availability for one condition is not evidence for another. Ask what is being proposed and which human evidence supports that exact use.
What human methylene blue studies found
There is human research outside methemoglobinemia. A small randomized, blinded study enrolled 26 healthy adults and examined brain imaging and cognitive tasks before and about an hour after oral methylene blue or placebo. It found changes in functional imaging and improved performance on a memory-retrieval task 2.
That is a preliminary experimental finding. It does not establish sustained improvement in everyday concentration, treatment of chronic fatigue or a longevity benefit. The trial also did not establish the efficacy of combining methylene blue with an NAD+ infusion, NMN or NR. Its short assessment window is particularly important when a service is marketed as a recurring program.
Another controlled study tested methylene blue after exposure training in people with claustrophobic fear. Outcomes depended on the success of that training; participants with more residual fear could fare worse 3. This illustrates how a context-specific learning effect should not become a blanket claim that a substance improves brain function.
Why an NAD+ increase does not prove synergy
A trial can show that an NAD+ precursor changes a blood measurement without establishing the benefit someone hopes to feel. For example, a short randomized NMN study increased circulating NAD+ but did not improve its measured physical-performance outcomes 4.
Combining mechanisms is a hypothesis, rather than a shortcut around this distinction. Demonstrating synergy would require a suitable human comparison: the combination against its individual components and an appropriate control, using meaningful outcomes and adequate follow-up. Neither the methylene blue studies above nor the NMN trial supplies that comparison.
Our NAD+ evidence guide separates laboratory findings from clinical outcomes. The same standard should apply when an additional ingredient is introduced.
Different claims require different evidence
| Question | What the cited evidence supports |
|---|---|
| Approved use | PROVAYBLUE injection for acquired methemoglobinemia |
| Oral cognition research | Small, short or context-specific experiments |
| NAD+ precursor research | Some blood biomarker changes; outcomes depend on the study |
| NAD+ plus methylene blue | The cited studies do not establish combination efficacy |
Medication interactions deserve a separate review
Methylene blue can inhibit monoamine oxidase A, an enzyme involved in serotonin metabolism. Laboratory work demonstrated this action using purified human enzyme 5. That mechanism helps explain why it deserves a medication review rather than being treated as an ordinary vitamin.
PROVAYBLUE carries a boxed warning for potentially serious or fatal serotonin syndrome with serotonergic medicines and opioids. Its interaction information also identifies dextromethorphan, an ingredient in some cough medicines 1. An interaction assessment must include prescriptions, nonprescription products and supplements, not just antidepressants.
The injection label provides important clinical warnings, but it does not quantify the risk of every oral wellness formulation or establish a safe self-treatment threshold. Do not infer safety from a lower advertised concentration or from a route difference. A prescriber or pharmacist needs the exact formulation and medication history. Do not stop prescribed medication or follow an online washout schedule to make a wellness combination possible.
G6PD deficiency and other clinical considerations
PROVAYBLUE is contraindicated in people with G6PD deficiency because of the risk of hemolytic anemia, and in those with severe hypersensitivity to methylene blue or related dyes 1. Disclosing medical conditions is part of a treatment assessment, not an optional checkout detail.
A claim that NAD+ supports energy metabolism does not show that it neutralizes methylene blue's contraindications or interactions. These are separate questions. Our NAD+ side-effect guide covers the NAD+ side of a discussion; it cannot substitute for reviewing an additional drug.
Questions worth resolving before paying
Ask for the precise ingredients, route, product identity, proposed purpose and responsible clinician. Request the human study supporting the combination itself. Check whether a quoted benefit was actually measured in a relevant population, rather than inferred from a brain image, blood NAD+ result or animal experiment.
Also distinguish a supervised research protocol from a commercial service. A package containing two biologically active ingredients can add cost and uncertainty without adding proven benefit. For the specific claim of feeling more energetic, read what NAD+ energy studies show before comparing packages. The evidence currently supports careful evaluation of the separate interventions, not a proven NAD+–methylene blue wellness combination.
Frequently asked questions
Can you take NAD+ and methylene blue together?
The studies reviewed here do not establish the benefits or safety of a general wellness combination. The exact products, route, medical history and other medicines require clinician or pharmacist review; NAD+ does not make methylene blue's interactions irrelevant.
Is methylene blue FDA approved for energy or longevity?
PROVAYBLUE is an FDA-approved IV product for acquired methemoglobinemia. That indication does not establish oral wellness products or combinations with NAD+ as treatments for energy, cognition or longevity.
Why do antidepressants and cough medicines matter?
Methylene blue has monoamine oxidase effects, and PROVAYBLUE warns about serotonin syndrome with serotonergic medicines and opioids. Its interaction information also identifies dextromethorphan. Provide a complete medication list and do not change prescribed medicines to accommodate a wellness product.
Does G6PD deficiency matter for methylene blue?
Yes. PROVAYBLUE is contraindicated in G6PD deficiency because it can cause hemolytic anemia. A different marketed product or the addition of NAD+ should not be assumed to remove that concern.
