Our verdict

PlexusDx offers NAD+ in three provider-selected forms—injection, nasal spray and Flex Dose Tablet—with the same stated plan price for each. The one-month rate is $179. The lower $149 and $129 averages belong to three- and six-month commitments, and the terms make clear that those plans are prepaid, so their totals are $447 and $774. Its optional genetics layer may appeal to someone already interested in testing, but it is not required to receive NAD+. The main purchase question is whether the longer commitment makes sense for an uncertain benefit, rather than whether adding a test makes the program more personalized in principle.

NAD+ evidence

The site describes NAD+ pathways and an optional genetics layer, but also says the genetic test does not recommend or determine which peptides someone should use. A genetic association is not evidence that the selected NAD+ formulation improves symptoms. Injection, nasal and oral NAD+ also need separate evidence from oral precursor research.

Pricing and fees

Advertised price
Starting at $129/month
Payment terms
$179 month to month; $447 prepaid for three months or $774 prepaid for six months
Pricing details
The $129 and $149 figures are monthly averages of prepaid six- and three-month plans. Turning off renewal does not create a prorated refund of the current prepaid term.

Additional fees

  • The optional genetics add-on is $99 after the first month. It is not required for a prescription.
  • The terms exclude outside testing, emergency or specialist services, some replacement and early-fill costs, and applicable taxes unless specifically included.

Prices are from PlexusDx's own website and can change without notice.

Who it may suit

May suit you if

  • A shopper comparing several NAD+ delivery routes with one published price ladder and clinician-selected formulation.
  • Someone who wants a month-to-month $179 option and understands how the lower prepaid rates affect flexibility.
  • A patient interested in optional genetics information who understands it is a separate educational layer rather than a demonstrated guarantee of treatment response.

Consider another option if

  • Someone who wants the $129 rate without paying for six months upfront.
  • A person who needs certainty that a genetic report identifies an effective NAD+ treatment, or an Arkansas resident specifically seeking the nasal formulation.
  • You need a general satisfaction refund after changing your mind about a prepaid term.

What’s included

Consultation
Independent clinical care is provided by Lone Star Virtual Health using Bask Health. The terms include clinically required synchronous consultations as well as routine provider care.
Lab testing
The NAD+ page says no blood work is required at intake. Optional testing may be recommended according to history, and outside laboratory work is not included unless stated.
Follow-up care
Monthly provider check-ins and provider-reviewed refills are included. Formulation changes remain a clinical decision. Auto-renewal continues for the same plan term unless canceled or paused.
Clinical support
The program includes platform support and ongoing provider oversight. Self-service plan management is available; emailed cancellation requests should be submitted at least three business days before renewal for processing.
Supplies
The terms include applicable injection supplies. The clinician selects the formulation and prescription; there is no published price surcharge simply for choosing another listed form.
Shipping
Shipping is included and fulfilled by a licensed partner pharmacy. The terms separate the prescription term from dispensing cadence, which can depend on formulation and pharmacy. Ask how your prepaid supply will be split into shipments.

Limitations and policy details

The low $129 headline is a six-month commitment paid upfront. The terms preserve some rights when treatment is medically discontinued, but simple cancellation is not a satisfaction guarantee. The $99 genetics offer should also be judged separately; an optional test does not establish that a longer NAD+ subscription is clinically worthwhile.

Other considerations

  • The $129 rate requires $774 prepaid for six months.
  • Canceling renewal does not automatically refund unused prepaid time.
  • Nasal NAD+ is unavailable in Arkansas.
  • The genetic add-on is optional and does not establish treatment effectiveness.
