There is no rigorous human trial behind subcutaneous or intramuscular NAD+ — not an outcome study, not even a published pharmacokinetic one — so there is no validated dose for it to hand you, and a tool that produced one would be inventing it. Search “NAD+ injection dosage chart” and you will find a dozen tidy tables: 50 mg for beginners, 100 mg for maintenance, 200 mg for “advanced protocols.” They agree with each other closely enough to look like consensus. They are not consensus — they are the same clinic marketing copied between med-spa blogs, and none of them cites a dose-ranging study, because none exists for this route.
The contrast with the oral side of the category is stark, and it is the single most useful thing to understand before spending money here. Oral NR has formal ascending-dose pharmacokinetics in humans; oral NMN has randomized dose-finding trials at 300, 600 and 900 mg/day. Both reliably raise blood NAD+ — and even they have struggled to convert that into how people feel. The injected route has neither the trials nor the biomarker data, and it costs several times more. Concentration first. A vial arrives as a powder with a milligram strength on it. Whatever volume of bacteriostatic water the pharmacy tells you to add sets the concentration: total mg ÷ mL of water = mg/mL. Nothing about the syringe means anything until that number exists.
Then the draw. Dose ÷ concentration = volume in mL, and a U-100 insulin syringe is marked so 100 units = 1 mL. So: 500 mg vial + 5 mL water = 100 mg/mL → a 50 mg dose is 0.5 mL → the 50-unit mark. Five of those a week is 250 mg/week, about 1,083 mg a month, and one 500 mg vial covers ten doses — roughly a fortnight.
The failure mode worth naming. The diluent volume, not the dose, is what moves the answer. Mix the same 500 mg vial with 10 mL instead of 5 and it becomes 50 mg/mL — so a 100 mg dose is now 2 mL, which is two full U-100 syringes, not one. The calculator flags that case rather than printing a number no barrel can hold. If your result and your pharmacy’s instructions disagree, the instructions win and the pharmacy gets a phone call.

- What is the correct NAD+ injection dosage?
- There isn't an established one, and any chart that presents a number as clinically validated is overstating what exists. No published randomized controlled trial — and no published human pharmacokinetic study — has ever set a subcutaneous or intramuscular NAD+ dose. The figures circulating online come from clinic and telehealth marketing, where 50–100 mg per subcutaneous dose, two to five times a week, is what providers commonly advertise. Treat that as the range the industry sells, not as a validated protocol, and take your actual dose from the clinician who prescribed it.
- How do I convert an NAD+ dose in mg to units on an insulin syringe?
- Divide the vial's total milligrams by the millilitres of bacteriostatic water added to get the concentration in mg/mL. Then divide your dose in mg by that concentration to get the volume in mL, and multiply by 100, because a U-100 insulin syringe is marked so that 100 units equals 1 mL. Worked example: a 500 mg vial reconstituted with 5 mL of bacteriostatic water is 100 mg/mL, so a 50 mg dose is 0.5 mL, which is the 50-unit mark. The diluent volume is what drives the answer — the same 50 mg lands on a different mark if the vial was mixed with a different amount of water.
- How much bacteriostatic water do I add to a 500 mg NAD+ vial?
- Whatever your compounding pharmacy's instructions say — the volume is a decision the pharmacy makes, not a universal constant, and this calculator deliberately does not pick one for you. Common reconstitutions land at 100 mg/mL (5 mL into a 500 mg vial, or 10 mL into a 1000 mg vial) because that makes the syringe maths tidy, but a vial mixed to a different concentration is not wrong, it just changes the units you draw. If the printed instructions and the vial label disagree, ask the pharmacy before mixing anything.
- Does this calculator tell me how much NAD+ to inject?
- No, and that is deliberate. This site's own evidence review concludes there is no rigorous human trial behind injected NAD+ at all, so a tool that generated a personalized milligram figure would be manufacturing an authority that does not exist. The calculator takes the protocol your prescriber set and does the arithmetic on it: what it adds up to per week and per month, how long a vial lasts, and where the dose falls on the syringe.
- Is more NAD+ per injection better?
- There is no evidence that it is. The dose-response work that does exist is for oral precursors, where raising blood NAD+ plateaus and higher doses mostly buy cost and side effects rather than more benefit. For the injected route there is no dose-response data at all. What is well documented is that NAD+ given too fast provokes flushing, chest tightness, nausea and cramping, which is why IV infusions are stretched over hours and injected doses are kept small.
